Scorpion Bite
SCORPION Bite: Research on treatment modality. Scorpion bite is quite common in this region. However, proper treatment thereof is not yet available in most of the villages. Villagers by and large depend on Vaidus, Tantra / Mantra etc. in the absence of reliable treatment. A research for treatment of Scorpion Bite is undertaken at Walawalkar Hospital. Konkan coastal scorpions are cardio toxic. After the bite they may cause pulmonary oedema due to cardiac failure. Until now over 300 scorpion bite cases have been treated at the Hospital. 2-D Echocardiography is done on the day of admission, on 5th day, on 15th day & after 6 months. Research is being conducted on the treatment modality of the scorpion bite, taking into account the long-term effects thereof, on the patients.

Objectives:
Prazosin is supposed to be a physiological & pharmalogical antidote of the scorpion venom but we studied efficacy of Dobutamine in prazosin resistant cardiotoxic cases.
Design:
A retrospective analysis.
Setting:
This study was a retrospective analysis of 198 patients admitted from 1999 to 2004. We classified these patients clinically in Three groups mild, moderate & severe Envenomation. For treatment plan 4 groups were made. Where patients received Dobutamine + Prazosin / No Do + PR / No Pr + No Do / Do + No PR. Effect of these two drugs were compared on the basis of mortality ratio in all 4 groups.
Results:
Out of 198 patients 107 patients developed Cardiotoxicity. Out of them 70 had received Prazosin as a first line of treatment. Out of 70, 29 needed Dobutamine to treat Cardiac complications. Mortality was highest in only Prazosin, No Dobutamine group and lowest in Prazosin, Dobutamine group.
Conclusion:
Prazosin alone is not effective in preventing and treating cardiac complications but a synthetic analogue of catecholamine i.e. Dobutamine is needed to save the life.
